Design a request form with an approval flow

Designs a request form and a simple approval flow for one kind of request, with fields, approver roles and time limits, as a draft for policy review.

Task: Design a request form and an approval flow for [type of request, such as purchase requests or time off]. Context: Requests come from [who submits] and are approved by [roles that approve]. We handle about [number] requests a month. Today the process is [current process] and the problem is [main problem]. The form will be built in [form or approval tool]. Rules: - List at most 10 form fields, mark each required or optional, and say why each is needed. Leave out fields that nobody will use. - Define the approval path by role, with a limit above which the next role must approve, using placeholders for amounts or thresholds. - Give each step a time limit in working days and say what happens when nobody responds. - Collect no more personal data than the decision needs. Use roles, not named people, and make no decisions about named individuals. - Treat the flow as a draft to check against company policy, internal control rules and local law before use. Result: A table of the fields with the columns field, required and reason, followed by the approval path as a numbered list of at most 6 steps.
