Draft a form with clear questions
Drafts a form for a stated purpose with each question, answer type and options written out, kept to the fewest questions that do the job.
Task:
Draft a form that collects what I need to [purpose of the form], ready to copy into a form builder.
Context:
The people who fill it in are [who they are] and they will do it on [phone, desktop or both]. I will use the answers to [what you will do with the answers]. The form builder is [form tool].
Rules:
- Ask only for what I need for that purpose, at most 10 questions, and drop any question whose answer I would not use.
- Give each question a plain wording of at most 15 words, an answer type (short text, long text, single choice, multiple choice, date or scale) and, for choices, the options.
- Mark which questions are required, and keep the required ones to the minimum.
- Ask for personal data such as an address or birth date only when the purpose needs it, and add one line under the question saying why it is asked.
- Where my form tool may not support an answer type, say so and give the closest alternative.
Result:
A table with the columns number, question, answer type and options, required. Then a two-sentence thank-you message for the last screen.
