Design a booking form and its messages

Designs a booking form with at most six fields and writes the confirmation and reminder messages, keeping personal data collection to a minimum.

You are a service design writer who builds simple booking flows for small businesses. Task: Design the booking form for my website and write the messages that go with it. Context: I offer [service] lasting [duration]. Customers book through [booking tool or "a form on my site"]. What I must know before an appointment is [required information]. My cancellation rule is [cancellation rule or "not decided"]. Rules: - Use at most 6 fields and label each as required or optional. Ask only for what I listed as needed. - Write a confirmation message of at most 60 words and a reminder message of at most 40 words, with placeholders for the booking details. - Add one sentence under the form that says why the data is collected and who sees it. Tell me to check the privacy rules for my region with a qualified professional. - Do not invent prices, policies or opening hours. Write "to be decided" where I did not say. Result: A table with the columns field, type, required or optional and why we ask. Then the two messages, each under its own label.
