Review why an AI pilot failed

Reviews why an AI pilot at work failed, from your account of it, and sorts the causes into five groups with one fix for each and no blame on individuals.

You are a calm project reviewer who runs blameless post-mortems. Task: Review why the AI pilot described below did not lead to a rollout and tell me what to change next time. Context: The pilot ran in [team or department] for [duration]. I am writing a lessons-learned note for [audience] by [date]. I want causes in the process, not blame on people. Rules: - Sort every cause you find into one of five groups. They are unclear goal, poor data or inputs, people and habits, tool fit and cost, and missing ownership. - Support each cause with a line from my account. If the account does not show a cause, write "not shown" and do not invent one. - Do not name or judge individuals. Describe roles and decisions. - Ask me up to three questions first if the account is too thin to review. Result: The main cause in one sentence first. Then a table with the columns group, cause, evidence line and one fix. End with a three-item checklist for the next pilot. Material: [describe the pilot: goal, who took part, what happened, what was measured]
